Notes
- ACA column: UnitedHealthcare’s 30 Marketplace states for plan year 2026. Coverage is by county, so not every ZIP code in a listed state has a plan. 2027 lists arrive with Open Enrollment on Nov 1, 2026.
- All other columns: UnitedHealthOne’s broker product availability page. State rules also set term limits; for example, Virginia caps short term at 92 days and pauses sales during Open Enrollment.
- Short Term vs. TriTerm: every TriTerm state also sells Short Term. Delaware, Illinois, Indiana, Kansas, Michigan, Minnesota, Montana, Nevada, Ohio, Pennsylvania, South Dakota, Virginia, Wisconsin and Wyoming sell Short Term but not TriTerm.
- Not listed: hospital indemnity and term life have no published state list. Check by ZIP code.
Important information
- Product availability varies by state. Not every product is sold in every state, state rules change term lengths, and ACA Marketplace coverage is by county, so not every ZIP code in a listed state has a plan.
- Not ACA coverage. Short Term Medical, TriTerm Medical and fixed-benefit plans are medically underwritten, can deny you, and are not Minimum Essential Coverage under the Affordable Care Act. They don’t qualify for premium subsidies. Fixed-benefit and supplemental plans are not a substitute for major medical coverage.
- Preexisting conditions. Short Term Medical excludes preexisting conditions, generally anything diagnosed or treated in the prior 24 months (12 months in Indiana and Texas). Fixed-benefit plans exclude them. TriTerm Medical covers them only after 12 months of coverage.
- Some short term plans are sold only as association group coverage through FACT, an independent consumer association (about $4/month).
- A federal rule effective Sept 1, 2024 limits how long short term plans can run. Check what your state allows today.
- Plan figures are summarized from Golden Rule brochures and UnitedHealthcare One plan pages (state lists published 2023–2026; some plan figures from a 2020 brochure). Benefits, deductibles, prices and availability can change, so confirm on a ZIP-code quote before you buy. This page is a summary for comparison, not a policy or a quote.
- UnitedHealthcare One plans are mostly underwritten by Golden Rule Insurance Company. UnitedHealthcare, UnitedHealthcare One and Golden Rule are trademarks of their respective owners.
